Experience in Marine & Oil Terminal/ Port operation as Person in Charge in the port operation and facilities, Head of Marine and Terminal Product Movement, Act Superintendent, such responsible for Marine & Terminal Installation, maintenance and operation; Ship shore – Shore Loading Procedure; Oil and gas processing; Calibration such equipment; Co-work with official government.

Mooring Master/ Head of Mooring Master at Unocal (Indonesian–American Oil Company) at Santan Terminal. Dalam penerapan SMK3 yang baik, suatu perusahaan disyaratkan untuk melakukan identifikasi bahaya dan risiko keselamatan kerja yang mungkin muncul akibat proses dan alat kerjanya.  Agar dapat mengetahui apakah prinsip kerja aman yang diterapkan telah memenuhi, perlu dilakukan Identifikasi Bahaya dan Penilaian Risiko (IBPR). Terdapat beberapa metode yang dapat digunakan untuk melakukan penilaian dan pengendalian risiko seperti misalnya, identifikasi bahaya, penilaian dan manajemen risiko, HAZOPS, FMEA, CHECKLIST, What If Analysis, Fault Tree Analysis, Event Tree Analysis, Cause Analysis Consequences, dll. Beberapa metode yang umum digunakan di berbagai perusahaan sebagai best practice akan dibahas dalam materi pelatihan, termasuk Job Safety Analysis (JSA).

Bahaya dan risiko ada dimana-mana di sekeliling kita. Jenis bahaya dan tingkat risiko tergantung dari kondisi lingkungan yang dihadapi termasuk di lingkungan kerja. Jenis bahaya dan tingkat risiko dari setiap tahapan proses dalam suatu proses industri adalah spesifik. Tidak semua pekerja mampu mengenali bahaya dan risiko dari pekerjaan yang mereka lakukan. Mengetahui jenis bahaya dan tingkat risiko di lingkungan kerja adalah kunci pokok untuk dapat mengendalikan bahaya dan risiko tersebut agar tidak menjadi malapetaka atau kecelakaan yang tidak diinginkan. Berbagai teknik telah dikembangkan untuk mengidentifikasi bahaya dan kajian risiko sehingga dapat dikembangkan sistem atau program pengendalian bahaya dan risiko ditempat kerja. HIRAC dan JSA adalah metode yang banyak digunakan dalam melakukan identifikasi bahaya ditempat kerja.

Pelatihan ini akan memberikan pemahaman kepada peserta pelatihan tentang pendekatan praktis untuk mengidentifikasi bahaya keselamatan dan kesehatan kerja sesuai dengan masing-masing proses bisnis dan lingkungan kerja tiap perusahaan, dan risiko potensial yang terdapat didalamnya sebagai dasar dalam meminimalisasi dan mengurangi risiko melalui pendekatan yang sistematis. Selain itu, Pelatihan ini dirancang untuk memberikan kompetensi kepada peserta pelatihan bagaimana prosedur dalam pembuatan HIRAC dan JSA serta membuat HIRAC dan JSA dengan menggunakan format standar.

TUJUAN
Pelatihan HIRARC ini diselenggarakan selama 2 hari yang bertujuan untuk mengetahui pentingnya melaksanakan HIRARC, saat perlu dilakukannya dan mampu mengenali serta  mengendalikan bahaya dan risiko di tempat kerja untuk mencegah kecelakaan dan kerugian. Setelah mengikuti pelatihan ini peserta diharapkan dapat membuat dan menerapkan HIRAC dan JSA diperusahaan sehingga kecelakaan dapat dicegah dan ditanggulangi dengan baik sesuai dengan aturan yang berlaku diperusahaan.

Dr.Ir. Sutanto Hadisupadmo.MT.,IPM is a lecturer at Physics Engineering Bandung Institute of Technology (ITB), and has been as a Head of Laboratory of Industrial Instrumentation at ITB. He is former Director of Instrumentation Center and Technical Services of ITB. His education started at Engineering Physics, Bandung Institute of Technology (ITB-1977), then at Diplome Universite in Energetique, Universite de Prepignan, France (1985), Magister Engineering in Instrumentation and Control (ITB) and Doctor (ITB). He has over 20 years of experience doing training, consulting work in instrumentation, measurement, safety and process control and Mr. Sutanto has trained many training/courses for many companies in Indonesia such as PT. TOTAL Indonesie, Pertamina, PT. Petrokimia Gresik, PT. Badak LNG, PT. Pupuk Iskandar Muda, PT. Pupuk Kaltim, PT. Jaya Ancol and many others. He has certificates in Health, Safety, and Environment (HSE) from Depnakertrans RI/Institutions and Universities in Indonesia, institutions and Universities in UK, and National Safety Council. Experienced in setting up Health and Safety Management System documentation and its application. Experienced internal lead auditor SMK3 in PT. Pupuk Sriwidjaja, and External lead auditor SMK3 for Indonesian Association Fertilizer Producer. He has been teaching industrial training classes for many classes related to topics of his competencies, HSE. HAZOPS is one of some significant, systematic and valuable methods to implement critical assessment, evaluation and examination of process and engineering of both new and old processing plant installations. HAZOP Studies have become the cornerstone of today’s process safety in design and modification work. Menurut Peraturan Pemerintah No. 74 tahun 2001 dan No. 18 tahun 1999, Bahan Berbahaya dan Beracun (B3) dan limbah B3 adalah bahan/limbah yang karena sifat, konsentrasi atau jumlahnya berbahaya bagi tenaga kerja dan dapat membahayakan lingkungan. Upaya pengelolaan dan pengendalian B3 dan limbah B3 harus dilakukan untuk mencegah dan mengedalikan dampak lingkungan yang akan muncul sebagai konsekuensi atas penggunaan bahan ini.

Berbagai upaya dengan segala jenis metode yang diterapkan  dari setiap lembaga/industri mungkin sudah rutin dilakukan dan dikendalikan yang berhubungan dengan B3 ini, tetapi mungkin tetap saja memperoleh tantangan dari pihak-pihak yang berkepentingan. Tak jarang, aparat hukum dan media massa turut terlibat dengan isu B3 ini yang berakibat pada konflik in interest. Bagaimana mengatasi keadaan non-teknis ini menjadi peluang bagi pengusaha untuk meminimalkan budget pengelolaan yang sesuai dengan peraturan??

Pelatihan ini sedikit akan membahas B3, limbah B3 dan porsi terbesarnya adalah untuk mengetahui teknik pengendalian B3 ditinjau dari segi hokum. OVERVIEW:
Hydrocarbon processing industry is categorized as a high risk industry due to the hazards containing either in the material processed or the operations used in its facilities. Because of its vulnerability, a simple event, either man-made or natural one may lead to significant disasters. Flixborough, Bhopal, Piper Alpha disasters are classic examples of major accident causing serious loss of life and/or property. A deep understanding on safety related aspects of the materials and processes or operations implemented in hydrocarbon facilities is therefore critical to operate this kind of industry in safe manner and preventing it to go wrong. The course provides a thorough understanding of the HAZOP techniques and experience of their application. Legislation and best practice require companies to proactively manage health and safety of employees and public. The HAZOP Technique is a widely used method of identifying hazards and operability problems, providing the foundation for the risk assessment process. HAZOP can reduce pre and post start-up modifications and improve productivity and product quality. On course, the lectures are underpinned by syndicate exercises.

DESKRIPSI

Ketika perusahaan menerapkan perundangan dan perturan K3 (seperti SMK3, OHSAS 18001, ISO 31000, IS00 2200 dan lain-lain) maka pemantauan lingkungan kerja penting dilakukan untuk tujuan spesifik antara lain: Memenuhi persyaratan internal dan eksternal, bahan untuk perancangan, pengendalian proses, pembuktian dalam proses hukum, penelitian dll. Agar hasil pemantauan -baik yang dilakukan oleh pihak internal maupun eksternal perusahaan- tersebut dapat dipertanggungjawabkan objektivitas dan validasinya maka pemantauan haruslah dilakukan dengan prinsip, teknik, sumber daya manusia yang kompeten dan prosedur yang benar dengan memperhatikan tujuan pemantauan, baku mutu lingkungan, sampling (prosedur, teknik, lokasi pengambilan dan penanganan), satuan-satuan dalam pemantauan dll. Hasil pemantauan seperti inilah yang dapat digunakan untuk melihat kesesuaian (compliance) antara kinerja lingkungan kerja perusahaan dengan peraturan yang berlaku dan untuk mengukur kinerja program lingkungan kerja, sehingga dapat ditentukan tindak lanjut dan perbaikan yang perlu dilakukan oleh perusahaan. Hasil pemantauan ini digunakan untuk membuat laporan pemamtauan lingkungan kerja baik mandatory Laporan ke Dinas Tenaga Kerja ( provinsi, Kab/kota) dan Kemnakertrans (pusat)) maupun yang voluntary (laporan tahunan ke publik). Disamping itu juga dapat digunakan sebagai bukti hukum dalam menghadapi complaint dan tuntutan dari stakeholders lingkungan (pemerintah, LSM, masyarakat, asuransi, dll).

TRAINING INSTRUCTOR
Bambang Pamudji, B.Sc.,SE. has much real experiences, he is former Senior Health and Safety staff at PT. Pupuk Sriwidjaja,PUSRI(Urea Fertilizer Manufacturer) Palembang. He was born in Pasuruan East Java, 14 March 1950. He got his B.Sc in Public Health from APK Surabaya in 1971 and was graduated from Economic Management Sriwidjaja University, UNSRI, Palembang in 1996. He has certificates in Health, Safety, and Environment (HSE) from Depnakertrans RI/Institutions and Universities in Indonesia, institutions and Universities in UK, and National Safety Council. Experienced in setting up Health and Safety Management System documentation and its application. Experienced internal lead auditor SMK3 in PT. Pupuk Sriwidjaja, and External lead auditor SMK3 for Indonesian Association Fertilizer Producer. He has been teaching industrial training classes for many classes related to topics of his competencies, HSE. Many participants from many companies in Indonesia have participated in his training classes.

Introduction
This 3 days course will focus on the technique of conducting a HAZOP Studies as applied to Oil & gas, Petrochemical, Chemical and other process plant An open discussion forum is available at the end of the course to discuss participant problem in the area of HAZOPs

Objective :

  • To provide participants with basic understanding & knowledge of HAZOP as one of the PHA (Process Hazard Analysis) methodology, in identifying and mitigate potential facilities process design deficiencies and hazardous deviation from the original design intent in the area of Oil and Gas, Petrochemical, Chemical and other process plant which could lead to the hazard condition and en danger the health, safety, and environmental, and also causes serious economic losses.
  • At the end of the courses, participants are expected to understand HAZOP principals in their operation, be able to perform & implement simple HAZOP study
