Menurut Peraturan Pemerintah No. 74 tahun 2001 dan No. 18 tahun 1999, Bahan Berbahaya dan Beracun (B3) dan limbah B3 adalah bahan/limbah yang karena sifat, konsentrasi atau jumlahnya berbahaya bagi tenaga kerja dan dapat membahayakan lingkungan. Upaya pengelolaan dan pengendalian B3 dan limbah B3 harus dilakukan untuk mencegah dan mengedalikan dampak lingkungan yang akan muncul sebagai konsekuensi atas penggunaan bahan ini.

Berbagai upaya dengan segala jenis metode yang diterapkan  dari setiap lembaga/industri mungkin sudah rutin dilakukan dan dikendalikan yang berhubungan dengan B3 ini, tetapi mungkin tetap saja memperoleh tantangan dari pihak-pihak yang berkepentingan. Tak jarang, aparat hukum dan media massa turut terlibat dengan isu B3 ini yang berakibat pada konflik in interest. Bagaimana mengatasi keadaan non-teknis ini menjadi peluang bagi pengusaha untuk meminimalkan budget pengelolaan yang sesuai dengan peraturan??

Pelatihan ini sedikit akan membahas B3, limbah B3 dan porsi terbesarnya adalah untuk mengetahui teknik pengendalian B3 ditinjau dari segi hokum. Dalam pelatihan ini juga akan diberikan panduan teknis supaya tenaga kerja di lapangan dapat melakukan penanganan B3 dengan aman, efisien dan berkenan dengan peraturan yang berlaku sehingga beban financial dapat dikontrol dengan lebih terperinci. Selama session pelatihan, peserta juga akan memperoleh contoh-contoh kasus untuk memperdalam pengetahuan dalam mengelolah B3 dan limbahnya.

Target peserta: Staff/Operator produksi, Environment Officer, K3/safety officer, Staff purchasing, petugas laboratorium dan profesional lainnya yang terlibat dalam pengelolaan administrasi dan teknis B3 dan limbah B3.

Metode Pelatihan
Pelatihan ini akan dilakukan dengan metode interaktif yang menyerupai semi-workshop dimana peserta akan langsung diajak mendiskusikan setiap topic yang disajikan, diberi contoh aplikasi terkini dan telah teruji, serta sharing informasi tentang hal-hal yang bersifat praktis dan administrasi dalkam usaha pengelolaan B3 dan limbah B3.

Encep Heryani adalah lulusan dari Universitas Pakuan Bogor Jurusan biologi dan AKTA IV Jakarta dan menyandang gelar terakhir Magister Sains (M.Si). Berangkat karir dari bawah mulai tahun 1986 sebagai analis mikrobilogi selama 17 tahun di perusahaan baby milk formula yang ada di Citeureup Bogor, kemudian bekerja di perusahaan susu infant formula lisensi Jepang menjabat Supervisor Quality Assurance selama 3 tahun dan lanjut memimpin Quality selama 8 tahun dan terakhir Memimpin Produksi di perusahaan makanan kesehatan untuk orang diabet sampai tahun 2018.

Di Perusahaanya menjadi karyawan teladan dan fasilitator terbaik continuos improvement, meraih prestasi terbaik dalam kontes practical problem solving, the best trainer and author education and training. Dan sebagai anggota PERMI (Perhimpunan Mikrobiologi Indonesia)

Gas tersting onvolves testing for oxygen, toxic and flammable gases using portable gas detection equipment and is an integral part of establishing safe system of work. The authorised gas testing (AGT) e-learning course covers: gas testing procedure and practices, the procedural requirements for gas testing, the teory of combustion, characteristics of gas mixtures and the use of portable gas testing equipment. He was born in Pasuruan East Java, 14 March 1950. He got his B.Sc in Public Health from APK Surabaya in 1971 and was graduated from Economic Management Sriwidjaja University, UNSRI, Palembang in 1996. He has certificates in Health, Safety, and Environment (HSE) from Depnakertrans RI/Institutions and Universities in Indonesia, institutions and Universities in UK, and National Safety Council. Experienced in setting up Health and Safety Management System documentation and its application. Experienced internal lead auditor SMK3 in PT. Pupuk Sriwidjaja, and External lead auditor SMK3 for Indonesian Association Fertilizer Producer. He has been teaching industrial training classes for many classes related to topics of his competencies, HSE. Many participants from many companies in Indonesia have participated in his training classes

M), under sponsorship or study assignment of PT Stanvac Indonesia and he also studied at University of Pembangunan Nasional (UPN) Veteran, Yogyakarta (1986-1991), graduated as Petroleum Engineer (Ir.), under sponsorship or study assignment of Huffco/VICO Indonesia. His work experiences is 39 years in oil and gas companies and 1 year in geothermal, detailly as follows:

HSE Work Practices At Confined Space (BANDUNG)

(Bekerja Yang Sehat, Selamat dan Peduli Lingkungan pada Ruangan Terbatas/Tertutup)

INTRODUCTION :
Ada beberapa kecelakaan fatal (orang meninggal dunia) yang pernah terjadi dilapangan eksplorasi dan produksi (EP) minyak dan gas bumi di Indonesia dimana “sang korban” lagi memasuki dan/atau bekerja diruang tertutup/terbatas (Confined Space), antara lain :

  1. Di terminal/jetty (Kaltim, sekitar tahun 1980an), siang hari, setelah barge berisi solar dipompakan ketanki solar didarat, seorang teknisi masuk kedalam barge untuk memeriksa kondisi bagian dalam dinding barge (compartment), rekan-rekannya yang diluar barge heran karena teknisi tadi sudah +/-2 jam tidak muncul dari dalam barge. Kemudian petugas Safety memakai APD lengkap masuk kedalam barge/kompartemen, ternyata teknisi ditemukan sudah meninggal.
  2. Di Stasiun Pengumpul Pusat/SPP (Kaltim, sekitar tahun 1985an), tengah malam, seorang operator produksi naik keatas atap tanki produksi untuk mengukur/memastikan tinggi minyak dalam tanki jenis terapung (floating roof tank) kapasitas 10.000 barrel. Beberapa saat kemudian, rekannya dari ruang operator memanggilnya berulangkali via radio/handy talky tapi tidak ada menjawab. Kemudian 2 petugas Safety dengan APD lengkap menyusul naik ke atap tanki, ternyata operator tersebut ditemukan sudah meninggal diatas atap tanki terapung.
  3. Disumur minyak yang sedang ditest atau Well Testing (Kaltim, kwartal kedua tahun 2011), operator (“A”) bermaksud mengukur tinggi cairan dalam tanki dengan mengunakan tongkat atau stick, kemudian stick nya terjatuh masuk kedalam tanki, “A” masuk kedalam tanki untuk mengambil stick tadi, dan tidak bisa keluar, datang “B” masuk tanki untuk menolong “A”, juga tidak keluar, datang lagi “C” masuk untuk menolong, juga tidak bisa keluar dari tanki, datang lagi “D”. Ternyata 3 orang meninggal dunia (“A”, “B” dan “C”), sedangkan “D” pingsan dan masih bisa tertolong dirumahsakit.

Dari kejadian-kejadian fatal diatas membuktikan bahwa memasuki dan/atau bekerja dalam confined space adalah merupakan kegiatan yang mengandung bahaya dan risiko tinggi yang akan berakibat fatal bagi siapapun. Untuk itu diperlukan pengetahuan, training, pengalaman, latihan/praktek/simulasi bagaimana cara bekerja dalam confined space agar setiap petugas/orang tetap sehat, selamat dan tidak mencemari/merusak lingkungan disekitar confined space.

TRAINING OBJECTIVES :

Selama dan setelah mengikuti pelatihan ini, maka para peserta dapat :

  1. Mengetahui, memahami dan melaksanakan prinsip-prinsip dasar manajemen pengendalian rugi sehingga akibat dari suatu kerugian (seperti kecelakaan, kebakaran, pencemaran lingkungan, dll) bisa diminimalisir.
  2. Mengetahui, memahami dan melaksanakan Job Safety Analysis (JSA) dan Confined Space Entry & Work Permit untuk pekerjaan-pekerjaan yang terkait dengan Confined Space.
  3. Mengetahui,memahami dan melaksanakan “13 Minutes HIRARC” (Hazard Identification, Risk Assessment & Risk Control) untuk pekerjaan-pekerjaan yang terkait dengan Confined Space.
  4. Mengetahui, memahami dan mampu menggunakan alat-alat/peralatan/instrument yang dipakai dalam pekerjaan di Confined Space, antara lain seperti : Explosive Meter/Multi Gas Detector, SCBA, H2S Personal Monitor, APAR, Blower, dll.
  5. Mengetahui, memahami dan mampu memasuki dan/atau bekerja dalam Confined Space dengan cara dan peralatan yang benar dan sesuai, sehingga dirinya maupun orang lain, tetap sehat dan selamat serta tidak tejadi gangguan atau pencemaran terhadap lingkungan.

TRAINING MATERIAL OUTLINE :

  1. Prinsip-Prinsip Dasar Manajemen Pengendalian Rugi :
  • Sejarah/Kronologis istilah HSE, Loss Prevention, Loss Control.
  • Definisi : accident, incident, near miss, safety.
  • Accident Ratio Study ; Teori Gunung Es (Ice Berg).
  • Loss Causation Model ; Pre-Contact Control, Contact Control, Post-Contact Control.
  1. Ruang Tertutup/Terbatas atau Confined Space (Pengertian/Definisi; Jenis-Jenisnya ; Dll.).
  2. Pencegahan dan Penanggulangan Kebakaran Pada Confined Space :
  • Teori segitiga api ; Jenis-jenis/kelas kebakaran
  • Teknik pencegahan kebakaran, Media dan alat pemadam kebakaran, Teknik penanggulangan kebakaran
  1. Izin Memasuki dan Bekerja Dalam Confined Space (Confined Space Entry and Work Permit) :

Catatan : akan membahas formulir permit ini yang dipunyai oeh Tuan Rumah/User seperti terminologi-terminologinya, cara-cara pengisian formulir, dokumen-dokumen pendukung yang harus dilampirkan (seperti Work Order, Program kerja, JSA, dll), pihak-pihak yang terkait dalam proses work permit, dan lain sebagainya).

  1. Analisa Keselamatan Bekerja atau Job Safety Analysis (JSA) untuk pekerjaan didalam Confined Space. CATATAN : setelah diberi teori tentang JSA, maka peserta training akan dibagi dalam 4 atau 5 kelompok @ 4-5 orang/kelompok untuk membuat JSA untuk 4-5 jenis pekerjaan yang berbeda antara satu job dengan job lainnya :
  • Membersihkan/mengosongkan sludge dari dalam/dasar Tanki Produksi di SPP.
  • Memperbaiki/mengelas plat dasar tanki minyak yang rusak/bocor di SP.
  • Membersihkan bagian dalam tanki lumpur (Mud Tank) dilokasi Rig Pemboran.
  • Memperbaiki/mengelas dinding bagian dalam dari Separator 3 Fasa (3 Phase) di SP.
  • Memperbaiki/mengelas plat dasar tanki solar (Diesel Fuel Tank) di Terminal BBM.

Atau Tuan Rumah/User dipersilahkan juga untuk memberikan tugas-tugas Confined Space lainnya. Kemudian JSA ini dipresentasikan oleh masing-masing kelompok dan didiskusikan bersama.

  1. “13 Minutes HIRARC” (Hazard Identification, Risk Assessment & Risk Control) :
  • Identifikasi Bahaya-Bahaya yang potensial pada pekerjaan-pekerjaan dalam Confined Space.
  • Penilaian Risiko dari bahaya-bahaya potensial tersebut terhadap HSE, apakah termasuk Risiko Rendah (Low Risk), Risiko Menengah (Medium Risk) atau Risiko Tinggi (High Risk).
  • Pengendalian Risiko, bagaimana mengendalikan atau mengontrol risiko-risiko tersebut agar tidak terjadi kerugian-kerugian seperti penyakit akibat kerja, kecelakaan kerja, kebakaran, pencemaran lingkungan, dan lain sebagainya.

CATATAN : para peserta training akan dibagi dalam kelompok seperti pada butir 5 diatas dan diberi tugas membuat “13 Minutes HIRARC” untuk 5 jenis pekerjaan Confined Space seperti pada butir 5.1. sampai dengan 5.5.) kemudian dipresentasikan oleh masing-masing kelompok dan didiskusikan bersama.

  1. Peralatan-Peralatan dan Alat Pelindung Diri (APD) untuk memasuki dan bekerja dalam Confined Space, bagaimana cara bekerjanya dan bagaimana cara menggunakannya, antara lain : Explosive Meter/Multi Gas Detector, Self Contained Breathing Apparatus (SCBA), Respiratory Protection Equipment, Personal H2S Monitor, Safety/Gas Explosion Proof Flashlight, Safety Belt, Full Body Harness, Bug Blower, APAR Dry Powder/Dry Chemical Kelas ABC ukuran 20 Lbs atau 10 Kg, Wheeled Fire Extinguisher Dry Powder/Dry Chemical Kelas ABC ukuran 150 Lbs, dan lain sebagainya.
  2. Pemutaran film/video yang terkait dengan Confined Space termasuk beberapa Case Study.
  3. Praktek atau latihan/simulasi “Memasuki dan Bekerja Dalam Confined Space” (pada hari ketiga training atau dari jam 8 sd 11.30 pagi, jam 12 – 13.00 istirahat, jam 13 sd 16.00 kembali kekelas, bila tidak memungkinkan kembali kekelas, maka bisa tetap dilapangan untuk mengevaluasi praktek tsb). Tuan Rumah/User menyediakan fasilitas dan peralatan-peralatan, antara lain sebagai berikut :
  • Satu tanki kosong (bentuk silinder atau segiempat) atau Separator/FWKO yang IDLE (TIDAK DIPAKAI, kapasitas terserah asal 1 – 3 orang bisa masuk dalam keadaan berdiri tegak. Lokasinya DIUSAHAKAN JANGAN YANG DI SP (Stasiun Pengumpul) ATAU SPP (Stasiun Pengumpul Pusat), MUNGKIN YANG ADA DI JUNK YARD.
  • Peralatan-peralatan dan APD seperti pada butir 7 ditambah tali tambang (manila rope) ukuran diameter 1 inchi & panjang 10 meter serta Kotak P3K.

CATATAN : dalam latihan ini Fasilitator Training akan dibantu seorang Asisten. Diskusi2 dan Tanya jawab akan dilakukan pada saat latihan praktek ini. Siang harinya semua kembali kedalam kelas untuk diskusi lebih lanjut dan evaluasi pelatihan, Post-Test, dll.

He has certificates in Health, Safety, and Environment (HSE) from Depnakertrans RI/Institutions and Universities in Indonesia, institutions and Universities in UK, and National Safety Council. Experienced in setting up Health and Safety Management System documentation and its application. Experienced internal lead auditor SMK3 in PT. Pupuk Sriwidjaja, and External lead auditor SMK3 for Indonesian Association Fertilizer Producer. He has been teaching industrial training classes for many classes related to topics of his competencies, HSE. Many participants from many companies in Indonesia have participated in his training classes.

The function of a pressure safety valve is to protect pressure vessels, piping systems, exchanger, boiler and other equipment from pressures exceeding their design pressure by more that a fixed predetermined amount. The permissible amount of overpressure is covered by various codes and is a function of the type of equipment and the conditions causing the overpressure. Failure of safety valve can cause cathastropic accident. Periodically inspection, testing and repair is one of the most important element that the safety valve work properly.This course explains how safety valve and relief functions and how to maintains and ensure long life operation.

OVERVIEW:
Hydrocarbon processing industry is categorized as a high risk industry due to the hazards containing either in the material processed or the operations used in its facilities. Because of its vulnerability, a simple event, either man-made or natural one may lead to significant disasters. Flixborough, Bhopal, Piper Alpha disasters are classic examples of major accident causing serious loss of life and/or property. A deep understanding on safety related aspects of the materials and processes or operations implemented in hydrocarbon facilities is therefore critical to operate this kind of industry in safe manner and preventing it to go wrong. The course provides a thorough understanding of the HAZOP techniques and experience of their application. Legislation and best practice require companies to proactively manage health and safety of employees and public. The HAZOP Technique is a widely used method of identifying hazards and operability problems, providing the foundation for the risk assessment process. HAZOP can reduce pre and post start-up modifications and improve productivity and product quality. On course, the lectures are underpinned by syndicate exercises.

Latar Belakang
Without ergonomics, HSE management is not enough. Ungkapan ini telah disampaikan oleh berbagai pakar dan praktisi HSE (Health, Safety dan Enviroment) atau K3L. Berbagai permasalahan HSE sering muncul, mulai dari near-miss hingga kecelakaan fatal, karena tidak diperhatikannya berbagai faktor manusia di berbagai lini operasi perusahaan (misalnya logistik, produksi, maintenance, pekerjaan kantor, dsb). Sentuhan ergonomi diperlukan dalam manajemen HSE.

Sayangnya, ergonomi sering disalah-artikan. Ergonomi dianggap mahal dan hanya dikaitkan dengan aspek kenyamanan kerja atau bagian kecil dari aspek industrial hygiene.  Padahal, tujuan ergonomi selaras dengan tujuan manajemen HSE yakni untuk menciptakan sistem kerja yang aman, sehat dan bahkan lebih produktif.  Dengan ergonomi, sistem kerja dipastikan dirancang sedemikian rupa dengan memperhatikan variasi pekerja dalam hal kemampuan dan keterbatasan (fisik, psikis, dan sosio-teknis). Semua rancangan dilakukan dengan pendekatan human-centered design (HCD), baik pada alat, mesin, tempat kerja, prosedur kerja, sistem informasi dan sistem organisasi.  Yang tentu semua itu akan membawa manfaat bagi bisnis perusahaan, karena ergonomic is economic.

Kenyataan di industri, banyak sekali sistem kerja yang tidak dirancang secara ergonomi yang harus menjadi perhatian dalam manajemen HSE. Kondisi berikut menunjukkan beberapa indikasi berikut:

  1. Sering terjadi accident atau near-accident
  2. Pekerja sering melakukan kesalahan (human error)
  3. Pekerja mengeluhkan adanya nyeri atau sakit pada leher, bahu, punggung, atau pinggang
  4. Pekerja terlalu cepat lelah dan butuh istirahat yang panjang
  5. Alat kerja atau mesin yang tidak sesuai dengan fisik pekerja
  6. Posisi kerja yang sering membungkuk dan menjangkau
  7. Lingkungan kerja yang tidak teratur, bising, pengap, atau redup
  8. Pekerja mengeluhkan beban kerja (fisik dan mental) yang berlebihan
  9. Komitmen kerja yang rendah
  10. Rendahnya partisipasi pekerja dalam sistem sumbang saran

Berpengalaman dalam Sistem Manajemen Chain Of Custody (CoC) atau lacak Balak   selama 11 tahun. Standar CoC yang dikuasai adalah standar FSC (Forest Stewardship Council), Standar LEI (Lembaga Ekolabel Indonesia), Standar VLO (Verification Legal Origin – TFF FORM), standar VLO/VLC Smartwood Rain Forest Alliance, Standar VLK (Verifikasi Legaitas Kayu) Depertemen Kehutanan, Standar PEFC (Pan Europa Forest Certification). Selain itu diakui sebagai auditor dengan   dengan  dokumen nomor LEI-AP/04-0400,  Rain Forest Alliance Recognize, NO 473/T/NP/pusdiklat/2011. Mendapat  ilmu manajemen industri kehutanan   saat menjalani pendidikan S1 Fakultas Kehutanan di Institut Pertanian Bogor. Pengetahuan dan ilmu Sistem Manajemen diperdalam saat Kuliah Magister Manajemen (S2) di Institut Pertanian Bogor. There were many lost-time and fatality accidents during hot work jobs especially in oil/gas exploration and production activities. The accident causes were various such as human factors (inadequate knowledge, skill and training, etc.), job factors (inadequate supervision, procedures and proper equipment, etc.) and lack of management (inadequate HSE program, program standards and compliance to program standards, etc.). By attending and implementing consistently the contents of this training course, the above accidents can be prevented.

    • Hot-Tapping Jobs at Flammable Fluid Storage Tanks
    • Hot-Tapping Jobs at Flammable Fluid Flow Lines & Pipe Lines
    • Hot-Tapping Jobs at Flammable Fluid Equipment & Pressure Vessels (Separators, Heater Treaters, Free Water Knock Outs, Gas Boots/Gas Scrubber/Gas Dryer,etc.)
    • Storage Tank Cleaning, Ventilation and Repairing

PT Caltex/Chevron Pacific Indonesia (CPI) Sumatera Operations Area. He has also 18 years experience (1992-2010) as a Petroleum, HSE, Human Resources & Security Trainer both In-House and Public Trainings.
