PENDAHULUAN:
Turbin uap merupakan salah satu jenis penggerak mula yang banyak digunakan di dalam industri, antara lain sebagai penggerak mula generator listrik, pompa dan kompresor, serta industri proses. Hal yang menjadi pertimbangan dalam penggunaaan turbin uap yaitu sumber panas yang digunakan untuk menguapkan air bisa dari bahan bakar gas, cair, dan padat, efisiensi turbin uap lebih besar daripada turbin gas, dan perawatan yangmudah.Permasalahan yang muncul pada turbin uap adalah distribusi kecepatan dan distribusi tekanan disekitar nosel dan sudu gerak, penurunan entalpi yang terlalu besar yang mengakibatkan kecepatan tangensial pada ujung sudu pun akan menjadi sangat besar, dan material juga memegang peranan penting sebagai kendala atas kecepatan tangensial yang cukup tinggi.Untuk mendukung sistem produksi yang memadai diperlukan tenaga kerja yang handal atau peningkatan keahlian tenaga kerja profesi Operasi dan Perawatan Generator dan Turbin Uap.

Maridjo has taught many topics related to his expertise to many companies in Indonesia, such as predictive and preventive maintenance, lubrication system, diesel machine, pump, gas turbine, piping and pipeline, compressor, boiler, and many others. Many participants from companies have been trained and collaborated with him such as Chevron, Semen Gresik, semen Baturaja, Pertamina, Pusri, GGPC, employee of Qatar and lybia oil and gas company, UP IV Balongan pertamina, PT. BACKGROUND:
Electricity is accepted as a source of power without much thought to the hazards encountered. Some employees work with electricity directly. This is the case with engineers, electricians, electronic technicians and power line workers. Others, such as office workers and salespeople work with it indirectly. OSHA’s electrical standards address this serious workplace hazard which exposes employees to such dangers as electric shock, electrocution, fires and explosions. The objective of the standards is to minimize the potential hazards by specifying designcharacteristics of safety in use of electrical equipment and systems

Bambang Pamudji, B.Sc.,SE. has much real experiences, he is former Senior Health and Safety staff at PT. Pupuk Sriwidjaja,PUSRI(Urea Fertilizer Manufacturer) Palembang. He was born in Pasuruan East Java, 14 March 1950. He got his B.Sc in Public Health from APK Surabaya in 1971 and was graduated from Economic Management Sriwidjaja University, UNSRI, Palembang in 1996. He has certificates in Health, Safety, and Environment (HSE) from Depnakertrans RI/Institutions and Universities in Indonesia, institutions and Universities in UK, and National Safety Council. Experienced in setting up Health and Safety Management System documentation and its application. Experienced internal lead auditor SMK3 in PT. Pupuk Sriwidjaja, and External lead auditor SMK3 for Indonesian Association Fertilizer Producer. He has been teaching industrial training classes for many classes related to topics of his competencies, HSE. Many participants from many companies in Indonesia have participated in his training classes.

INTRODUCTION :
Operasi pemboran didarat (onshore) banyak sekali mengandung bahaya dan risiko, baik terhadap kesehatan (Health), keselamatan kerja (Safety) dan lingkungan (Environment) atau K3L / HSE, dimana yang akan terkena dampak bahaya dan risiko tersebut adalah tenaga kerja / manusia (People), perlengkapan / peralatan {Equipment / Tools), bahan-bahan (Materials) dan lingkungan hidup disekitar operasi pemboran darat. Standar K3L / HSE yang tinggi dalam operasi pemboran akan tercapai bila tenaga kerja / karyawannya terlatih, baik dalam masalah operasi pemboran maupun dalam aspek-aspek K3L / HSE. Selain itu yang tidak kalah pentingnya tentu saja perlengkapan / peralatan yang dipakai harus memenuhi standar operasi yang dirancang dan disusun dengan baik, dioperasikan dengan program pemboran yang memperhatikan aspek-aspek engineering / rekayasa yang baik pula. INTRODUCTION :
Historically, industrial pulp and paper production was a semi – automatic process which required a great deal of manual intervention and there are some types of exposures to potential health and safety hazards such as physical, chemical and biological hazards which may be expected in each process area and these hazards will affect to employees from other areas may also occur depending on weather conditions, proximity to sources of exposure, and whether they work in more than one process area ( e.g. , quality control, general labour pool and maintenance personnel ). Also there are some environmental issues such as air pollution issues, water pollution issues and solid wastes handling. Due to these health, safety and environmental hazards, the Management and all employees of paper and pulp industry should set-up and implement HSE Management System and Procedures to handle the above HSE aspects in preventing and reducing accidents and losses : harm to people, damage to property or equipment, loss to process and damage to environment.

INTRODUCTION :
There were many accidents in industrial transportation ( land, marine and air transportation ) due to some causes such as human factors ( operators, drivers, helpers, load masters, etc. ); equipment failures / malfunction (example : vehicles, mobile heavy equipment, crew boats, supply boats, tug boats, oil barges, helicopters, etc.) ; environment conditions and the last but not the least : INADEQUATE and LACK OF TRANSPORTATION SAFETY MANAGEMENT SYSTEM. The purpose of this safety management system is to ensure that transportation activities are well planned, organized actuated or directed and controlled so that risks are minimized and transportation is conducted in compliance with applicable Local, Government of Indonesia ( GOI ) or National and International Laws and Regulations as well as the Best Recognized Practices / Standards.

TRAINING OBJECTIVES :

Banyak kecelakaan kerja terjadi di sebuah industri di karenakan system safety yang tidak berjalan. Seseorang dengan begitu mudahnya melakukan perbaikan sebuat alat tanpa di lengkapi proses ijin kerja yang cukup sehingga sering terjadi kecelakaan.

Kebakaran mudah terjadi di area industri bilamana hot work permit system di abaikan. Orang bisa pingsan bahkan terjadi fatality, di sebabkan masuk ke dalam tanki / ruangan terbatas tanpa menjalankan confined space entry permit.

System kerja yang tidak tersusun menyebabkan operator / kontraktor sulit untuk menolak sebuah pekerjaan hot work / confined space work dari atasan yang besangkutan, sehingga besar kemungkinan bisa menimbulkan kecelakaan kerja.

Ahli K3 sebagaimana dimaksud dalam pasal 6 ayat (3) mempunyai tugas:

  • Membantu mengawasi pelaksanaan peraturan perundang-undangan bidang
    penanggulangan kebakaran;
  • Memberikan laporan kepada Menteri atau pejabat yang ditunjuk sesuai dengan peraturan perundangan yang berlaku;
  • Merahasiakan segala keterangan tentang rahasia perusahaan atau instansi yang
    didapat berhubungan dengan jabatannya;
  • Memimpin penanggulangan kebakaran sebelum mendapat bantuan dari instansi yang berwenang;
  • Menyusun program kerja atau kegiatan penanggulangan kebakaran;
  • Mengusulkan anggaran, sarana dan fasilitas penanggulangan kebakaran kepada pengurus.
  • Melakukan koordinasi dengan instansi terkait.

Syarat-syarat Ahli K3 spesialis penanggulangan kebakaran adalah:

  1. Sehat jasmani dan rohani;
  2. Pendidikan minimal D3 teknik;
  3. Bekerja pada perusahaan yang bersangkutan dengan masa kerja minimal 5 tahun;
  4. Telah mengikuti kursus teknis penanggulangan kebakaran tingkat dasar I, tingkat dasar II dan tingkat Ahli K3 Pratama (Kebakaran B) .
  5. Memiliki surat penunjukkan dari menteri atau pejabat yang ditunjuknya

Dalam melaksanakan tugasnya Ahli K3 spesialis penanggulangan kebakaran mempunyai wewenang:

  1. Memerintahkan, menghentikan dan menolak pelaksanaan pekerjaan yang dapat menimbulkan kebakaran dan peledakan;
  2. Meminta keterangan atau informasi mengenai pelaksanaan syarat-syarat K3 di bidang kebakaran di tempat kerja.

Permits to work are used to control high risk work activities, such as hot work, work at height, confined spaces work etc. Method statements are used in many industries, again as a control measure to detail the correct and safe sequence for work operation.

OBJECTIVES:
The purpose of this training  is to help in determining if a job requires a work permit due to ESS&H hazards, system complexities, or work coordination needs.  If the job already has an established work procedure or has been reviewed and defined as “Worker Planned” work due to the training and qualifications of the crafts person or technician, then the checklist is not needed.

He got his B.Sc in Public Health from APK Surabaya in 1971 and was graduated from Economic Management Sriwidjaja University, UNSRI, Palembang in 1996. He has certificates in Health, Safety, and Environment (HSE) from Depnakertrans RI/Institutions and Universities in Indonesia, institutions and Universities in UK, and National Safety Council. Experienced in setting up Health and Safety Management System documentation and its application. Experienced internal lead auditor SMK3 in PT. Pupuk Sriwidjaja, and External lead auditor SMK3 for Indonesian Association Fertilizer Producer. He has been teaching industrial training classes for many classes related to topics of his competencies, HSE. Experienced in setting up Health and Safety Management System documentation and its application. Experienced internal lead auditor SMK3 in PT. Pupuk Sriwidjaja, and External lead auditor SMK3 for Indonesian Association Fertilizer Producer. He has been teaching industrial training classes for many classes related to topics of his competencies, HSE. Many participants from many companies in Indonesia have participated in his training classes

OVERVIEW:
The Hazardous Waste Operations and Emergency Response Standard (HAZWOPER) applies to five distinct groups of employers and their employees. This includes any employees who are exposed or potentially exposed to hazardous substances — including hazardous waste — and who are engaged in one of the following operations such as :

  • Clean-up operations — required by a governmental body, whether federal, state, local, or other involving hazardous substances — that are conducted at uncontrolled hazardous waste sites;
  • Corrective actions involving clean-up operations at sites
  • Voluntary clean-up operations at sites recognized by federal, state, local, or other governmental body as uncontrolled hazardous waste sites;
  • Operations involving hazardous wastes that are conducted at treatment, storage, and disposal facilities regulation
  • Emergency response operations for releases of, or substantial threats of releases of, hazardous substances regardless of the location of the hazard.

This course covers broad issues pertaining to the hazard recognition at work sites. OSHA has developed the HAZWOPER program to protect the workers working at hazardous sites and devised extensive regulations to ensure their safety and health. This course, while identifying different types of hazards, also suggests possible precautions and protective measures to reduce or eliminate hazards at the work place. HAZWOPER 40 Hour is required for employees working on a project consisting of Uncontrolled Hazardous Waste Operation mandated by the Government. This course is specifically designed for workers who are involved in clean-up operations, voluntary clean-up operations, emergency response operations, and storage, disposal, or treatment of hazardous substances or uncontrolled hazardous waste sites. Topics include protection against hazardous chemicals, elimination of hazardous chemicals, safety of workers and the environment and OSHA regulations. This course covers topics included in 29 CFR 1910.120. Note: Workers must have 40 hrs of initial training before they may enter the site and at least three days of actual field experience under a trained, seasoned supervisor. **The online course meets the standard requirement of 40 hrs of initial training. The three days field experience under a trained, experienced supervisor is the responsibility of the student employer or potential employer.

  1. Understand the purpose of OSHA and its role in regulating occupational safety
  2. Use Site Characterization to establish problems that may exist in your workplace and measures that can be implemented to eliminate hazards
  3. Identify hazardous materials existent in the workplace and the possible methods, symptoms and preventative measures of exposure
  4. Encourage the use of Material Safety Data sheets (MSDS) to identify and properly handle hazardous materials
  5. Familiarize yourself with materials, compounds and mixtures that may present flammable, explosive, chemical or radiological hazards
  6. Emphasize the importance of personal protective equipment in limiting hazardous exposure
  7. Establish an effective Site Control Program to limit the risk of exposure to only those working in the hazardous work zone
  8. Implement procedures for treating workers in the event of hazardous exposure
